Responsibilities:
When you come to our office for an interview or to fill out an application, please be sure to bring with you:
- Social Security Card
- Proof of Citizenship (Passport, Permanent Resident Card, Certificate of Naturalization, or Photo ID and Birth Certificate)
- Home Health Aide license from the State of New Jersey(If applying for certified home health aide position. Not necessary for companion positions.)
- The names and phone numbers of two professional references. (Former employers)
- Certified Home Health Aides also need a physical, MMR, and TB test. If the TB test is positive you will need a chest x-ray report.
